THE QUALITY OF LIFE OF PATIENTS WITH CHRONIC KIDNEY DISEASE WHO UNDERGO HEMODIALYSIS IN RSUD CILACAP

Abstract

Hemodialysis (HD) is the most widely used process of kidney function replacement in Indonesia, including in RSUD Cilacap. People with CKD who undergo HD tend to experience changes in various aspects of life one of them related to quality of life. Patients can survive on HD therapy but still leave a number of important issues as a result of HD therapy. This study aims to study the quality of life of patients with GGK undergoing HD in RSUD Cilacap. This research use analytic observational design with cross sectional approach. The sampling technique uses purposive sampling and the sample size is determined by the Bernoulli formula. The quality of life of respondentswas assessed based on WHOQoL-BREF questionnaire consisting of 4 domains: physical health, psychological health, social relations and environmental health. Other aspects to be studied in this study are the primary causes of CKD, clinical manifestations of HD patients, therapy adherence and adequacy of HD. Data were analyzed using descriptive analysis. The results showed that hypertension was the main cause of CKD at RSUD Cilacap which reached 59%. Clinical manifestations showed mean Hb levels of respondents were 9,15±1,72 g/dL, serum creatinine 12,36±3.33 mg/dL, serum ureum before HD 140,52 ± 39,93 mg/dL and Saturation Transferin of 38,21±16,55%. As much 69,4% of respondents had normal BMI. Adequacy of HD represents as much as 73,5% achieving an optimal HD with an average value of URR of 69,64 ±15.07%. The aspect of blood pressure showed that the average respondent was in the condition of hypertension stage 1 with average systolic and diastolic pressure respectively 148,67±29,24 mmHg and 80,10±5,47 mmHg. In the design of hypertension therapy, respondents using a single furosemide, a combination of furosemide and clonidine and a combination of furosemide and valsartan have achieved blood pressure <140/90 mmHg. The quality of life of the respondents on the physical health dimension was mostly at the poor level (69,4%) while in the dimension of psychological health, social and environmental health of the majority on the medium level were 53,1%; 76,5% and 53,1% respectively. The quality of life of HD patients in RSUD Cilacap is generally at an unfavorable level of 57,1%.
